  • A faulty locking mechanism in the captain’s seat caused the throttle to slip to idle during takeoff, leading to a fatal stall.
  • The co-pilot shouted, “We’re losing thrust!” but was unable to regain control due to the captain’s reclined position.
  • FAA and EASA have ordered urgent checks on all Boeing 787 seats, as Air India grounds 12 similar aircraft ahead of Boeing’s promised redesign.

A preliminary report has linked the devastating Air India Boeing 787 crash, which claimed 270 lives, to a faulty locking mechanism in the captain’s seat.

According to investigators, the seat unexpectedly slid backward just after takeoff, causing the captain to inadvertently pull the throttle levers to idle.

The sudden reduction in engine power led to a stall, bringing the aircraft down just seconds later into a nearby building housing medical personnel.

The co-pilot, unable to access the controls due to the captain’s reclined position, was heard shouting, “We’re losing thrust!” at the 15-second mark. Data retrieved from the cockpit revealed a precise timeline:
• +12 seconds: Captain’s seat shifts back
• +15 seconds: Co-pilot alerts, “We’re losing thrust!”
• +26 seconds: Aircraft stalls at 214 feet

The crash resulted in the deaths of all passengers and crew, as well as several people inside the building. A British national is the only known survivor.

The report also highlighted the absence of critical safety features in the Boeing 787’s throttle system, including reverse-motion protection and weight-based lockouts. In response, both the FAA and EASA have ordered immediate inspections of all 787 pilot seats.

Air India has since grounded 12 aircraft with similar maintenance records, while Boeing has committed to redesigning the seat mechanism by 2026. Authorities say the final report is expected in July and have promised sweeping reforms to global aviation safety protocols.
